Current Department Staffing: Primary Care Service Chief; 2 Lead Physicians; 24.0 FTE PACT PCP’s and 3.0 FTE GAP Physicians; 2 GAP NP’s; 3 Admin Staff.
This position will report to the Section Chief of Primary Care.
Practice Environment Description:
Panel size averages 1100. This is a non-supervisory position.
- Highlights of this position include:
- Daily schedule: Monday – Friday, 8:00am – 4:30pm.
- Physicians will be assigned to a Patient Aligned Care Team (PACT).
- The PACT care model consists of the following: PCP, RN, LPN, and Clerk.
- Additional ancillary staff: Social Work, Pharmacy, Mental Health, and Physical Therapy are located within the clinic.

- Board Certified or Board Eligible.
- Family Practice or Internal Medicine Licensure.
