Baptist AgeWell is seeking a compassionate full-time Nurse Practitioner to join our Homecare team. This is a primary care role where you will visit patients at home or in other settings. You must be comfortable and have experience working with adults with chronic medical conditions in a homecare setting. Good bedside manner is key for success in this role. You must possess the interpersonal skills to work with patients and families during a pivotal time.

W hat You Can Expect
- This Nurse Practitioner will treat patients in their residence, home, ILF, or ALF.
- We provide Epic EHR System.
- A c ollaborative team with 4 other Advanced Practice Providers with Medical Assistants and support staff.
- Established geriatric patient panel with some palliative care patients.
- You will treat 6 to 7 patients a day.
- Hours are Monday through Friday from 8 AM to 5 PM.
- Call schedule is 2 times per year.
- The patient panel is aslo fed from Baptist Medical Center Nassau
- Compensation is $114K to $140K based on experience with the ability to increase your compensation based on our productivity model.

- Master’s degree - Nursing, required
