About the job: As a Mental Health Rehabilitation Worker, you will provide support and assistance to persons served in their homes and communities and help maximize the self-help skills of persons served in the areas of daily living, coping, boundaries, education, employment, housing, benefits, medication monitoring, and mental and physical health.
Essential job responsibilities include, but not limited to:
- Follow persons served’s Individual Program Plan.
- Be available to persons served to provide positive social interaction.
- Conduct appropriate activities in persons served’s home and community in conjunction with individual plans.
- Monitor, report and document all unusual behavior and overall medical condition of persons served.
- Communicate concerns, suggestions, and information to supervisors in a complete, accurate, prompt, respectful and timely manner.
- Complete, sign, date and submit all documentation in a timely manner.
- Assure all quarterlies are written, scheduled, held and submitted timely.
Qualifications:
Must be 18 years of age or as dictated by certification requirements and meet the following qualifications:
- has an associate of arts degree in one of the behavioral sciences or human services, or
- is a registered nurse without a bachelor’s degree, or
- who within the previous ten years has:
- three (3) years of life experience with serious and persistent mental illness;
- three (3) years of life experience as a primary caregiver to an adult with a serious mental illness or traumatic brain injury (TBI); or
- 4,000 hours of supervised paid work experience in the delivery of mental health service to adults with a serious mental illness (SMI) or TBI, or
Meets all the following:
- is fluent in the non-English language or competent of the culture of the ethnic group to which at least 50% of the mental health rehabilitative worker’s persons served belong; and
- received during the first 2000 hours of work, monthly documented individual clinical supervision by a mental health professional; and
- has 18 hours of documented field supervision by a mental health professional or practitioner during the first 160 hours of contact work with persons served, and at least six (6) hours of field supervision quarterly during the following year; and
- has review and co-signature of charting of person served contacts during field supervision by a mental health progression or practitioner; and
- has 40 hours of additional continuing education on mental health topics during the first year of employment in the mental health field.
Must successfully complete 30 hours of training during the previous two-year period in, at least but not limited to, the following areas:
- Person served rights
- Person served - centered individual treatment planning
- Behavioral terminology
- Mental illness
- Co-occurring mental illness and substance abuse
- Psychotropic medications and side effects
- Functional assessment
- Local community resources
- Adult vulnerability
- Person Served confidentiality/HIPAA per MN Statues
