Job Summary
Prairie Grove Health and Rehab is seeking a dedicated and skilled Wound Care Nurse Manager to join our healthcare team. The ideal candidate will have a strong background in nursing with specialized knowledge in wound care management. This role involves assessing, planning, and implementing care for patients with complex wounds, ensuring optimal healing and recovery. The Wound Care Nurse will collaborate with interdisciplinary teams to provide high-quality care in various settings, including long-term care facilities and hospitals. On call one week out of the month. Bonuses offered.
Responsibilities
- Conduct comprehensive assessments of patients' wounds and overall health status.
- Develop individualized care plans based on assessment findings and evidence-based practices.
- Perform wound care treatments, including dressing changes, debridement, and application of topical agents.
- Monitor and document patients' progress, vital signs, and response to treatments.
- Educate patients and their families on wound care techniques and prevention strategies.
- Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care.
- Maintain accurate records using electronic health record systems such as Athenahealth.
- Participate in quality improvement initiatives related to wound care management.
- Provide triage support for patients presenting with acute wound issues.
Qualifications
- Valid nursing license (RN or LPN) in the state of practice.
- Experience in long-term care or hospital medicine preferred.
- Strong knowledge of physiology related to wound healing processes.
- Proficiency in performing venipuncture and monitoring vital signs.
- Familiarity with working in a Level II Trauma Center is a plus.
- Ability to lift patients as needed while maintaining safety protocols.
- Excellent communication skills and ability to educate patients effectively.
- Previous experience in nursing homes or similar settings is advantageous.
- Certification in wound care management is preferred but not required.

Pay: $26.54 - $28.00 per hour
