What are the responsibilities and job description for the Hospice Care Partner/Caregiver position at Prairie Senior Cottages?
This position will serve patients in Steele, Waseca, and Freeborn County. Travel is required.
We are looking for Hospice Care Partners to work 20-32 hours per week, including every other weekend and Holiday.

Responsibilities:
- Provide patients with assistance for activities of daily living (walking, bathing, bathroom needs, etc.)
- Perform light housekeeping duties
· Be a comforting and supportive companion for hospice patients and their loved ones
· Follow individualized care plans for patients
- Coordinate with all members of the hospice team
- Report changes to patient condition to the hospice nurse
- Report any unusual incidents
- Adhere to organization’s documentation and reporting standards
- Act quickly and responsibly in cases of emergency
Skills/Qualifications:
- Knowledge of housekeeping activities
- Willingness to adhere to health and safety standards
- Respectful and compassionate
- Good time management skills
- Ability to work independently
- Outstanding communication and interpersonal skills
- Strong ethics
- Physical endurance
- Valid driver’s license, transportation, and auto insurance
- Minimum of 1 year of caregiving experience (Preferred)
- CNA certification (Preferred)
- High school diploma or GED
