Hospice Care Partner/Caregiver

This position will serve patients in Steele, Waseca, and Freeborn County. Travel is required.

We are looking for Hospice Care Partners to work 20-32 hours per week, including every other weekend and Holiday.

You should be able to follow instructions and perform a variety of tasks to help our patients. Hospice car partners must have a valid driver’s license and reliable transportation in order to provide care wherever our patient’s call home. Availability:

• Part-Time Days/Evenings, Alternating Weekends/Holidays
• Full-Time Days/Evenings, Alternating Weekends/Holidays

Responsibilities:

• Provide patients with assistance for activities of daily living (walking, bathing, bathroom needs, etc.)
• Perform light housekeeping duties

· Be a comforting and supportive companion for hospice patients and their loved ones

· Follow individualized care plans for patients

• Coordinate with all members of the hospice team
• Report changes to patient condition to the hospice nurse
• Report any unusual incidents
• Adhere to organization’s documentation and reporting standards
• Act quickly and responsibly in cases of emergency

Skills/Qualifications:

• Knowledge of housekeeping activities
• Willingness to adhere to health and safety standards
• Respectful and compassionate
• Good time management skills
• Ability to work independently
• Outstanding communication and interpersonal skills
• Strong ethics
• Physical endurance
• Valid driver’s license, transportation, and auto insurance
• Minimum of 1 year of caregiving experience (Preferred)
• CNA certification (Preferred)
• High school diploma or GED
