The responsibilities of the Care Plan Coordinator include, but are not limited to :
Coordinate in conjunction with the M.D.S. Coordinator the completion of each resident's care plan in compliance with all State and Federal requirements.
Develop, write and implement a written plan of care that incorporates all I.D.T. elements to assure all resident care needs are known and met.
Ensure the needed changes / updates to each resident's plan of care are completed in a timely fashion.
Ensure education for all staff members of the care plan process and their role in implementation.
Excellent pay and benefit package available.
Monday-Friday 8AM-4 : 30PM
On call required every 5th-6th weekend
