Key Responsibilities:
- Assess individual needs and provide necessary services through communication, service coordination linkage with provider agencies, service monitoring, and individual advocacy.
- Schedule and conduct intake/PUNS interviews in response to referrals, gathering initial information as required under the PAS Manual.
- Complete the PAS process following designated procedures, utilizing assistance from supervisors when appropriate, to determine appropriateness and current availability of services.
- Provide linkage to other community services for individuals found to be ineligible for PAS/Individual Service Coordination services.
- Advocate on behalf of the individual and/or family to assist the individual in obtaining services to which he/she is eligible and has chosen.
- Review individual's medical, residential, vocational, educational, legal, and financial needs and individual choices through ISC Visiting Notes, as required.
- Schedule and complete Discovery and Person-Centered Plan process annually, or as required.
- Accurately and thoroughly document various services delivered and maintain individual files as specified by program and agency guidelines.
- Function in a problem-solving capacity with provider agencies regarding service coordination, following the ISC Conflict Resolution guideline.
- See all waiver individuals on an as-required basis and complete program requirements set forth in the waiver manual.
- Coordinate specialized services for eligible Bogard individuals as needed and submit summary reports to external agencies as appropriate.
- Monitor services provided to Bogard class members on a monthly basis and advocate for any unmet needs.
- Submit all required documents to Program Managers for payroll preparation.
- Participate in scheduled all-staff meetings and mandated training sessions as recommended by the Executive Director and/or Program Manager.
- Alert the Program Assistant of PSCI to problematic issues, trends, or service gaps.
