As a Patient Centered Medical Home (PCMH) Level 3, ensure the excellence of the PCMH program as well as the HRSA health center program, both of which work hand-in-hand with our Accountable Care Organization (ACO).

Responsibilities
- Develops and implements the organization’s quality improvement plan in accordance with the mission and strategic goals of the organization, federal laws and regulations.
- Develops and implements systems, policies, and procedures for the identification, collection, and analysis of performance measurement data.
- Collects and summarizes performance data, identifies opportunities for improvement, monitors outcomes.
- Prepares and presents an Annual Quality Report to the Board of Directors.
- Teaches a culture of quality and safety concepts to staff during orientation, committee/departmental meetings and annually.
- Organizes and assists with the Peer Review Process.
- Participates in the Community Health Assessment with other community collaborators.
- Participates in other clinic committees such as Emergency Preparedness, Risk Management, Grants Management, Infection Control, OSHA, etc.)
- Assists with monitoring and ensuring HRSA requirement for quality is compliant.
- Assists with the annual Uniform Data System report.
Qualifications
- LPN or RN
- Active Kansas License
