Praxis Health is launching a cutting-edge healthcare clinic, and we’re searching for an experienced Panel Manager to join our advanced primary care team. This is a high-impact, in-house role for a licensed or certified clinical professional with a strong background in care coordination and population health management. The ideal candidate is detail-oriented, collaborative, and passionate about improving outcomes for complex patient populations.
Certification Required: LPN or Highly Skilled MALocation: In-House Only (No Remote)
Key Responsibilities
1. Population Health Collaboration
Coordinate with centralized PXH/Regence teams to drive performance in:
HEDIS metric tracking
Transitional Care Management (TCM)
Annual Wellness Visit (AWV) completion rates
Hierarchical Condition Category (HCC) performance
2. Transitional Care Management (TCM)
Conduct intake for post-discharge patients and coordinate timely office visits
Ensure high-risk cases are included in the Interdisciplinary Team (IDT) agenda for review
3. High Emergency Room Utilization Oversight
Monitor and manage patients identified on the PRIA/PXH high ER-utilizer lists
4. Care Coordination Leadership
Lead daily AM huddles to align care teams on panel management priorities
Prepare and manage the agenda for weekly IDT meetings

Minimum Qualifications
Required Education
- High school diploma
- Licensed Practical Nurse (LPN) or Highly Skilled MA
- Basic Healthcare BLS certification.
- Solid understanding of medical terminology and managed care.
Required Experience
Population health program involvement or leadership
HEDIS metrics tracking and improvement
Outpatient primary care setting experience
Management of complex care patients
Preferred Experience
Behavioral health exposure or experience
Wound care knowledge
Familiarity with diabetes and insulin pump management

Physical Demands:
- Manual dexterity for office machine operation including computer and calculator; stooping, bending to handle files and supplies, or sitting for extended periods of time.
- Stress can be triggered by multiple staff demands and deadlines.
