We are a home health and hospice agency providing services in patient homes in 22 counties throughout Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, and resident care apartment complexes. Preceptor strives to provide high quality care through our multifaceted programs including home health, hospice, palliative care, and outpatient services. We have branch offices located in Germantown, Fond du Lac, Waukesha, and Appleton.
We are currently in need of a Part-Time Social Worker for our Home Care and Hospice Divisions
in our Milwaukee, Waukesha, and Racine County territories.
Our clients reside in assisted living facilities, their private home, or wherever home may be.
We bring our services to our clients.

Help provide resources and support to patients and their families.
This position allows you to think outside the box when supporting our patients, families, facilities, and our hospice and home health teams.
Qualifications for the right Social Worker:
Social Worker means an individual who is currently licensed to practice social work in the State of Wisconsin
Master’s Degree preferred. Must be a graduate from a school of social work accredited by the Council on Social Work Education
Bachelor's Degree accepted when accompanied by one year of hospice experience.
Hospice and Home Health experience preferred, but not necessary. We are willing to train the suitable candidate to set them up for success.
Great communication skills, both verbal and in writing
Compassionate and hard working
Driver's license and auto insurance
Position Responsibilities:
As a Home Health and Hospice Social Worker, you will provide wide range of psychosocial services to terminally ill patients and their families.
HOSPICE
Participate in the hospice admission process with the hospice nurse
Regularly assess the patient and family psychosocial needs as well as assess the pre-bereavement/anticipatory grief needs.
Collaborate with the hospice IDG, patient, family, and facility where the patient may reside
Create and implement the Social Work Plan of Care provisions to meet the patient needs
Regularly re-evaluate the patient's and family's psychosocial needs.
Attend weekly IDG meetings and meetings
Participate in on-call rotation with the social work and chaplain team members as assigned
HOME HEALTH
Provide information on resources to families
Help with future planning
One-on-one education about health and wellness, tailored to patients' needs, home routines and goals.
