Summary:
The home health coordinator is responsible for assisting the Account Executives with educating the community on the services offered by the agency and maintaining contacts within the medical community. The home health coordinator provides coordination of community resources for clients served by the Agency.
Essential Duties and Responsibilities: (Note: Other Duties May Be Assigned)
- Pre-screen patients from physicians, hospitals, long-term care facilities and assisted living facilities for home health needs, eligibility and status, when referral is concurrent with discharge.
- Maintain healthcare contacts within the assigned territory. Maintain contact with hospitals, physicians, physician’s staff, discharge planners, social workers.
- Serve as a liaison between patients, physicians and agency staff to ensure that the patient’s needs are being met.
- Make presentations to current and prospective clients to educate them on the agency’s services.
- Deliver orders to physician’s offices for signature and ensure timely return of the orders.
- Communicate with each referral source to ensure that the expectations of the referral sources and the needs of the patient are being met.
- Provide timely, accurate completion of reports, worksheets, and information as requested by the Administrator/designee.
- Participate in company-sponsored programs and meetings and maintain a networking base in the community through trade associations, business organizations and local association meetings.
- Maintain knowledge of the agency’s policies and procedures, and Medicare regulations that are applicable to home health care.
Qualifications:
- Able to communicate effectively with customers
- Able to work cooperatively as a member of a team
- Able to work independently, make decisions, and problem solve
Education and/or Experience:
- 2 years of sales experience in home care and/or health-related field preferred
