Job Description

Community Living Supports (CLS) Team Lead will function as part of a multi-disciplinary team to provide assessment of recreation and leisure skills, treatment plan, and treatment. He/she will implement, supervise, and coordinate CARF Community Integration Programs. He/she will provide communication across, within, and between the Preferred team, including front line and program management staff and maintain a professional relationship with clients while modeling and guiding appropriate social interaction. The Team Lead will be responsible for conducting assessments and developing goals for individuals, completing billing and process notes, and other communications with contracting agencies. Primary job function is to develop specific programming plans and educate patients on the use of leisure activities to promote recover and/or rehabilitation to meet goals outlined by their PCP. Perform other duties as assigned.

Job Duties/Knowledge:

  • Handling office tasks, such as filing, generating reports as needed, and setting up for meetings
  • Answer and directing phone calls
  • Utilization of online system, ensuring scheduling is up to date
  • Maintaining filing system of employee and client records.
  • Produce and distribute correspondence memos, letters, faxes, and forms
  • Directing/interacting with Preferred Supports' clients, their families, and community members
  • Working with clients as scheduled/as needed
  • Establish open lines of communication for individuals with mental or emotional needs.
  • Requires basic understanding of mental illness, developmental stages, physical and emotional needs, life cycle changes, comfort and care appropriate to the age of the patient served.
  • Establishes and maintains a safe environment for patient care.
  • Review and implement treatment plans (IPOS, BTP, OT/PT).
  • Plan, organize, and conduct a program through the application of goal oriented, purposeful activity in the assessment and treatment of individuals with psychological, physical, and developmental disabilities.
  • Identifies supplies and materials needed for activities.
  • Utilizes individual and/or group activity interventions to bring about positive changes in client's behavior, be-it cognitive, social, emotional, or physical.
  • Evaluates each client's progress and the effectiveness of treatment, and records results on daily progress notes, following documentation policies.
  • Recognizes when the client has achieved goals or has achieved maximum benefit from services, and when necessary, modifies treatment plan accordingly.
  • Provide leadership and supervision to all staff that are employed in the Muskegon Individual CLS Program.
  • Coordinate with all staff that are employed in the Muskegon Individual CLS Program.

Qualifications

  • Previous experience in mental health counseling or other related fields.
  • Familiarity with community mental health resources.
  • Ability to build rapport with clients and staff
  • Excellent written and verbal communication skills
  • Successful completion of required training.
  • Current Michigan Driver's License, auto insurance and transportation and a driving record meeting Preferred's driving policy.
  • Clear criminal record.
  • Employee must be in good standing (evaluation/employment history, corrective action, etc.) to be considered for internal transfer. Employees must be off of probation and have successfully participated as a group lead staff for at least one year.
  • Must have ability to use hands for repetitive simple grasping when writing, using computer, answering phone, copying, and other related duties.
  • Frequent walking, frequent lifting of up to 25 pounds and occasional lifting of up to 50 pounds, standing for long periods of time.
  • Must be able to effectively work in a variety of stressful situations, while conducting activities with clients. Special attention to detail and problem-solving abilities required.
  • Bachelor's degree in social work is recommended, but not required.
  • Must be able to work 32 hours per week.
  • Must be flexible in hours to be on a rotation of opening and/or closing the office around programming time and to be shared with other office staff.
  • Must be available for on-call/after hour emergencies.
