Key Responsibilities:

• Meet with clients to encourage service utilization and monitor their needs.

• Develop and maintain partnerships with community organizations to assist clients in accessing insurance, medical care, transportation, and other resources.

• Provide case management services to support clients' individualized needs.

• Assist clients with Medicaid enrollment or securing an appropriate insurance plan.

• Coordinate and track referrals to primary and specialty care providers.

• Re-engage clients who have not maintained medical treatment.

• Model professional behavior consistent with agency expectations and program philosophy.

• Perform other duties as assigned by the Clinical Supervisor, Program Director, Vice President, or Executive Team member.

Education and/or Experience Qualifications:

• Bachelor’s Degree in social work, counseling, psychology, psychiatric nursing, or a closely related field;
  OR

• Four (4) years of relevant, qualifying full-time experience in human service delivery, demonstrating skills in developing productive client relationships and navigating complex service systems.
  OR

• A combination of education and relevant, qualifying experience (substitution allowed: one year of experience per year of education).

Additional Qualifications:

• Knowledge of case management and rehabilitation methods as they relate to mental health and/or substance use disorders.

• Familiarity with community resources for mental health and substance use.

• Basic computer skills, including proficiency with Microsoft Word and Excel.

• Strong communication skills for verbal and written reports.
