POSITION SUMMARY :
A Home Care Coordinator executes and supports each clients' home care solution. This includes maintenance of ongoing communication and coordination with the referral, family members, client, healthcare providers and intake sources for all clients. The Home Care Coordinator shall plan for all home healthcare service coverage from therapy, nursing, caregivers, and companions, while ensuring that the patient / family are receiving excellent customer service.
