What are the responsibilities and job description for the Hospice Admissions Nurse (RN) Full Time position at Preferred Hospice?
We are a well-established hospice company looking to add to our growing team!! Preferred Hospice has 5 offices in Missouri serving over 65 counties in the state. Those offices are in Dexter, Farmington, St. Louis, Columbia, and Ozark.
We are currently looking for a Full Time RN to serve as an Admissions Nurse in our Farmington, MO office which is a very busy and fast-paced office!

JOB DUTIES:
The Admissions Nurse primarily coordinates and performs the initial evaluation of a new patient, determining if they meet hospice criteria, and completing the admission process. This includes contacting new referrals to set up the evaluation, explaining the hospice benefit and limitations, reviewing medication lists for hospice appropriate meds, ordering necessary DME, answering family and patient questions, etc.
The Admissions Nurse is typically the first hospice employee that a new patient and family meet. This nurse must have strong assessment skills and some knowledge of hospice rules and regulations. This nurse must represent our company well and respond quickly to new referrals.
The Admissions Nurse provides symptom management and skilled care to patients facing terminal illnesses as well and providing emotional support and education on death and dying.
The Admissions Nurse will assist with routine visits and on-call as needed.
This job provides a lot of autonomy as well as great team work and support from management and co-workers!
The Admissions Nurse works with the Interdisciplinary Team made up of RNs, LPNs, CNAs, Social Workers, Chaplains, Physicians, Nurse Practitioners, Bereavement Staff, Volunteers.
The office is open Monday through Friday, 8am to 5pm, with some call required. Hours can be flexible. Reliable transportation and car insurance required.
Health, dental, vision insurance available for FT along with 401K and optional coverage including cancer policies, short-term disability, etc.
Hourly rate plus mileage reimbursement. Recent increase in mileage reimbursement company-wide.
QUALIFICATIONS:
The successful candidate must be currently licensed in the State of Missouri as an RN and live within a commutable distance to Farmington.
We are looking for a nurse who exhibit a thorough knowledge of the holistic approach toward patient care and the nursing process that applies to hospice patients and families.
Home Health, Hospice, Long-Term Care experience preferred but not required.
Willing to train the right candidate. A gentle, loving heart is a must. Must be able to provide love and support to patients and families facing a terminal illness. Must be able to work as a team player and represent our company well in the community.
Computer and smart phone skills for EMR charting, email, apps for mileage computation, etc.
We prefer either professional or personal experience in death and grief.
