Treatment nurse responsibilities include:
1. Observes, assesses and reports resident condition/changes and documents accordingly.
2. Performs skin assessments of residents in accordance with facility policy/procedure/practice.
3. Completes Pressure Ulcer Risk Assessments upon admission and re-admission and when otherwise indicated.
4. Implements interventions according to resident’s risk factors for developing pressure ulcers and teaches staff, resident and family the importance of the interventions.
5. Implements and maintains established procedures relative to the skin management.
6. Provides consultation to Nursing Staff regarding pressure ulcer risk issues, interventions and wound care needs.
7. Prepares and administers treatments as per Physicians’ orders and clinical practice guidelines.
8. Receives, transcribes and executes physicians’ orders.
9. Manages skin tears, surgical wounds, etc.
10. Completes detailed documentation to include measurements per facility practice guidelines.
11. Confers with the facility’s interdisciplinary team regarding assessment evaluations and assist in planning and developing an individualized Care Plan for residents receiving wound care treatments.
12. Implements care plans as directed, as it relates to wound care, focusing on skin integrity and PU prevention.
13. Prepares required documentation for Director of Nursing in a timely and accurate manner.
14. Notifies MD of new skin issues, obtains treatment orders and implements changes as indicated.
15. Assists in collection of data relevant to resident assessment and evaluation.
16. Administers treatments, as prescribed, adhering to Infection Control principles and assists with Infection Control Surveillance.
17. Monitors functions and activities of subordinate staff.
18. Assists in the investigation of incidents and accidents as required.
19. Identifies and secures equipment and supplies necessary for the appropriate treatment of wounds or other skin conditions.
20. Assist with staffing needs on the floor and take call PRN.
