Job Summary :
Thrive Pathways LLC is seeking a dedicated CFSS Staff Member to support adults with Intellectual and Developmental Disabilities (IDD) and their families through the Coordinated Family Support Services (CFSS) program. This role involves providing personalized, community-based support to individuals and their families, helping them navigate services, enhance independent living skills, and strengthen family stability.
Key Responsibilities :
- Family-Centered Support : Assist adults with IDD and their families in developing skills and accessing resources to promote independence and well-being.
- Life Skills Coaching : Provide guidance in daily living activities, including financial management, personal care, social interactions, and self-advocacy.
- Service Coordination : Help families access generic & non-generic regional center services, community resources, and support systems.
- Community Engagement : Facilitate social-recreational activities and community outings to enhance participation and inclusion.
- Education & Advocacy : Empower individuals and families by providing information on their rights, available benefits, and self-determination options.
- Documentation & Reporting : Maintain accurate records of services provided, progress updates, and case notes in compliance with state and vendor agency requirements.
- Collaboration : Work closely with IRC service coordinators, program supervisor, and other support providers to ensure holistic and consistent care.
- Associate’s or Bachelor’s degree in Social Work, Psychology, Special Education, Human Services, or a related field .
- Experience working with individuals with IDD and / or family support services.
- Strong communication and interpersonal skills with a family-centered approach.
- Knowledge of regional center services and community resources.
- Ability to work independently, maintain confidentiality, and adhere to ethical guidelines.
- Basic computer and documentation skills for reporting and communication.
- Valid driver’s license, reliable transportation, and ability to travel within Riverside county as needed.
- Experience with Coordinated Family Support Services (CFSS) or working with persons with Intellectual and Developmental Disabilities.
- Bilingual skills (Spanish, ASL, or other languages commonly spoken in the community) are a plus.
- CPR / First Aid certification (or willingness to obtain).
