Licensed Clinical Social Worker - New Port Richey, FL
Licensed Clinical Social Workers (LCSW) / LMHC at Premier Community HealthCare Group (PCHG) provide patients with access to resources and behavioral health counseling. The LCSW provider serves patients who are confronting personal issues, such as mental illness, addiction, and abuse. We strive to serve our community in establishing, diagnosing, and treating acute and chronic conditions for strong mentally healthy communities. This role will collaborate with a multidisciplinary team to make a significant impact on the well-being of PCHG patients.
Essential Duties and Responsibilities :
- Maintain daily schedule to maximize productivity.
- Perform diagnostic evaluations, review medical history, and provide therapy for individuals.
- Communicate with other providers and school personnel, if necessary, to provide crisis intervention.
- Complete accurate documentation of patient encounter in Electronic Health Record (EHR) within 48 hours of the encounter.
- Review and address tasks daily, entering charges into system daily for billing.
- Educate patients on how to utilize community resources and how to resume life in community within limits of disability.
- Case Consult with fellow Premier LCSW’s, Clinical Director of Behavioral Health, Associate Medical Director of Behavioral Health as necessary in order to provide quality Counseling / Diagnostic services for Premier patients.
- Provide Counseling, Diagnostic, and Case Management services to children, teens and adults within the LCSW’s age range scope of competence.
- Ensure confidentiality and HIPAA compliance at all times
- Keep other care team members informed when situations occur that disrupts timely patient flow through site.
- Contribute to community give back initiatives and be an advocate for Premier’s mission
- Adhere to patient care needs and the core values of Premier Community HealthCare to provide best-in-class patient experience
- Work towards attaining department goals and drive Quality Improvement / Quality Assurance (QI / QA)
