Premier Genesee, a 160-bed skilled nursing facility with an active 40-bed rehab unit is seeking a Director of Social Work to oversee Familiarity with care planning, admission processes, discharge planning, MOLST, resident rights is required and at least one-year experience working in skilled nursing setting.
Premier Genesee, a 160-bed skilled nursing facility with an active 40-bed rehab unit. Premier Genesee is seeking a Director of Social Work to oversee the long term care units at the facility, the departmental staff, and assure adherence to all current NYS and federal regulations. This position provides a wonderful opportunity to make a meaningful impact in the lives of our residents and their families!
The ideal candidate will have work experience in the social services field, within a long term care family.
Qualifications include:
-Knowledge of MDS is essential
-Basic computer knowledge
-Knowledge of state regulation
-Long term care experience
Job Duties include:
-Serve as an advocate for our residents and function as a liaison between the resident, family member, and staff.
-Review and follow up with any concerns
-Conduct initial, quarterly, and annual social work assessments and develop individualized care plans
-Oversee care plan meetings with interdisciplinary staff, residents, and their families
-Assisting with the admission and discharge process
-Providing resident with referrals as needed
-Knowledge of resident’s rights
-Completing MOLST forms
-Completes all tasks as assigned by administration
-Complete social work portions of MDS
-Adhere to DOH standards
