LPN/RN Case Manager
The Case Manager is responsible for the coordination medical care for our Assisted Living residents.
We are looking for a professional nurse in assisted living may have the responsibility for assessment of potential residents to determine their suitability and safety living in an assisted living environment as well as assessing change in condition to determine if a resident needs a higher level of care.
Job Duties Include:
- Conducting health examinations
- Administering medication and monitoring the well-being of patients
- Corresponding with family members
- Organizing schedules of both patients and staff
- Creating wellness and exercise programs
- Coordinating with external healthcare professionals
This is a great opportunity for LPN or RN who wants to grow professionally.
Hourly position Monday through Friday.
Work hours are 8am to 4:30pm
This position will require some on call duties, unit coverage over weekends and/or holidays.
