Currently seeking skilled and driven MDS Coordinator to welcome our ever-growing team!

Coordinator Expectations:
- Maintains current MDS status of each patient/resident
- Communicates schedule to all other departments and families, as required
- Maintains proficiency in software programs for MDS completion and can utilize appropriate reports
- Monitors clinical systems for changes in patient/resident condition.

Requirements:
- Must be a college graduate; BSN is preferred
- LPN- RN
- Valid state license(s)
- Must have a minimum of two years direct hands-on nursing experience
- At least one-year MDS experience.
- Proof of Covid-19 Vaccination or exemption
