Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have opportunities for a Care Coordinator Supervisor for the North Side of Chicago.
The Case Coordination Unit (CCU) Care Coordinator Supervisor will oversee the quality work completed by their assigned Care Coordinators as well as manage the operations of their clerical support staff to ensure timely completion of assessments, care plans, and authorization of all necessary interventions.
Qualifications:
- Education: Minimum of a BA/BS degree in health or social sciences, social work, or health service administration. Any annual policy and regulatory requirements for licensure must be completed.
Work Experience:
- Have at least two years’ experience in health or human services.
- This experience shall include one year of supervisory experience or program experience, which is defined as assessment, provision, and/or authorization of formal services for the elderly.
- Must be knowledgeable of IDOA regulations for the Community Care Program,
- Strong organization, leadership and communication skills
- Experience utilizing computerized programs/systems (i.e. MS Word, Excel, PowerPoint, CMIS, eCCPIS)
Essential Job Responsibilities:
1. Audit review and sign off for all Determination of Need/Eligibility Comprehensive or Screen Assessment completed by each assigned Care Coordinator.
2. Responsible to ensure timely completion of the care planning process and development of Care Plans based on assessments for Assigned Care Coordinators and Care Coordination Assistants.
3. Required to enter all systems data into state systems timely and ensure all authorizations for CCP services are completed timely by support staff.
4. Provide oversight regarding the management of complaints and grievances related to services. Ensure timely responses are provided to enrollees and proper documentation completed.
5. Documented provision of training on all IDOA and Area Agency on Aging Policies, procedures, and case management techniques.
6. Conducts annual performance evaluation of all assigned Care Coordinators and conduct chart audits for assigned Case Coordinators.
7. Works collaboratively with other providers, contractors, government entities and LTSS advocates to support the care coordination of the Enrollee.
8. All other duties as assigned.
