Qualifications for Nurse Case Manager :
Current unencumbered State of New Hampshire RN / LPN license
Knowledge of Medicare, Medicaid, PPS, PDPM, MDS and third-party payment
Knowledge of regulatory agencies / licensure standards
Must have minimum 5 years’ experience working in geriatrics with subacute experience
Must have strong clinical background
Case management experience strongly preferred
Responsibilities for Nurse Case Manager :
Monitoring level of Managed Care patients on admission and throughout stay
Obtaining prior approval for all Managed Care patients throughout the facility for transportation, therapy, etc.
Tracking Managed Care and Medicare days
Obtain pre-authorization as needed for special services and equipment
Ensuring sound clinical and financial outcomes
Attending family meetings
