Key Responsibilities

• Engage in interdisciplinary discussions and patient reviews
• Conduct evaluations for new admissions and assess potential discharges
• Communicate patients’ progress and updates to family members and facility staff
• Provide staff training on patient progress and the use of adaptive equipment
• Implement programming for wheelchair, splints, and other adaptive equipment needs
  
  

Required Skills

• Strong interpersonal and communication abilities
• Experienced in performing evaluations for new admissions
• Proficient in interdisciplinary collaboration and patient progress reporting
• Familiarity with programming adaptive equipment
  
  

Qualifications

• Licensed Physical Therapist
• Experience in skilled nursing facilities is preferred
• A genuine passion for working with senior residents

Compensation And Benefits
