- Conduct comprehensive evaluations of residents.
- Participate in care conferences to assess resident progress and needs.
- Develop and implement safe discharge plans in collaboration with patients and caregivers.
- Perform home evaluations as necessary to ensure a safe transition post-discharge.
- Implement effective seating systems to enhance resident comfort and safety.
- Strong evaluation and assessment capabilities.
- Excellent communication and interpersonal skills.
- Ability to collaborate effectively with patients, caregivers, and fellow team members.
- Valid Occupational Therapist license.
- Experience in skilled nursing facilities is preferred.
