Key Responsibilities
- Conduct thorough evaluations and assessments of patients.
- Participate in care conferences to address patient needs and progress.
- Develop and implement safe discharge plans in collaboration with patients and caregivers.
- Perform home evaluations to ensure a safe living environment for patients.
- Introduce and manage safe seating systems for residents.
- Proficient evaluation and assessment capabilities.
- Strong collaboration skills for effective teamwork in interdisciplinary settings.
