Key Responsibilities
- Conduct thorough evaluations of residents.
- Participate in care conferences to discuss resident progress.
- Develop and implement safe discharge plans in collaboration with patients and caregivers.
- Perform home evaluations to ensure safe transitions.
- Establish safe seating systems for residents.
- Exceptional communication and interpersonal abilities.
- Proficiency in executing comprehensive evaluations.
- Knowledge of discharge planning processes.
- Experience in conducting home evaluations and implementing safe seating systems.
- Degree in Occupational Therapy or a related discipline.
- Valid licensure to practice as an Occupational Therapist in New York State.
- Previous experience in skilled nursing or comparable settings is preferred.
