Key Responsibilities
- Conduct comprehensive evaluations of residents.
- Participate in care conferences to address resident needs.
- Facilitate safe discharge planning in collaboration with patients and caregivers.
- Perform home evaluations to ensure suitable environments for patients.
- Implement effective seating systems to ensure patient safety and comfort.
- Exceptional communication abilities.
- Proficient in assessing and evaluating patient needs.
- Knowledgeable about safe discharge planning processes.
- Capable of implementing secure seating systems.
- Collaborative approach to working with multidisciplinary teams.
- Valid state licensure as an Occupational Therapist.
- Experience in skilled nursing facilities is preferred.
- A strong passion for improving the quality of life for residents.
- Excellent interpersonal skills and the ability to engage effectively with residents and staff alike.
