Key Responsibilities
- Conduct thorough evaluations and assessments of residents.
- Actively participate in care conferences to discuss and monitor patient progress.
- Implement safe and effective discharge planning in collaboration with patients and their caregivers.
- Perform home evaluations as needed to ensure optimal resident care.
- Develop and establish safe seating systems tailored to individual resident needs.
- Proficient assessment and evaluation capabilities.
- Exceptional communication and interpersonal skills.
- Expertise in developing and executing tailored treatment plans.
- Knowledgeable in safe seating systems.
- Experience in discharge planning and executing home evaluations.
- A valid Occupational Therapist state license is required.
- Previous experience in a skilled nursing facility is preferred.
- A genuine passion for enhancing the quality of life for residents.
- A Bachelor’s or Master’s degree in Occupational Therapy from an accredited institution.
