Key Responsibilities
- Engage in interdisciplinary discussions and reviews of scheduled patients and treatment plans.
- Conduct evaluations for new admissions to ensure personalized care pathways.
- Assess potential discharges and effectively communicate related floor programs.
- Participate in staff training and provide updates on patient progress.
- Program adaptive equipment such as wheelchairs and splints, tailored to individual needs.
- Maintain open communication with families and facility staff regarding patient progress, updated goals, and any changes in status.
- Strong assessment and evaluation capabilities for new admissions.
- Excellent interpersonal and communication skills to liaise effectively with staff, families, and residents.
- Experience in programming adaptive equipment, including wheelchairs and splints.
- Proven ability to collaborate within a team-oriented environment.
- Valid state licensure as a Physical Therapist.
- Previous experience in a skilled nursing or rehabilitation setting is preferred.
- A Bachelor’s degree in Physical Therapy or a related field.
