Key Responsibilities
- Collaborate effectively with the therapy team and interdisciplinary teams to provide comprehensive resident care.
- Educate families about therapy processes and objectives to support resident recovery.
- Participate in facility in-services and educational opportunities to enhance skills.
- Engage in care planning conferences to contribute insights for resident care strategies.
- Conduct home assessments to facilitate safe discharge planning for residents.
- Excellent communication skills for effective teamwork and family education.
- Proficiency in conducting home assessments ensuring safe transitions for residents.
- Ability to actively participate in care planning and family education initiatives.
- Relevant experience in occupational therapy, preferably within a skilled nursing setting.
- An associate’s degree or higher in Occupational Therapy or a related field.
- Certification as an Occupational Therapy Assistant.
