RN Case Manager Home Health

Job Overview
We are seeking a compassionate and dedicated Registered Nurse to provide high-quality patient care in the comfort of patients' homes. Position compensation includes excellent benefit package with health insurance, earned time off, IRA with agency contribution, agency i-Phone, mileage reimbursement, and the most competitive market per visit rates! Position is salaried at the top range for the area.

We are looking for RN Case Managers to work Monday - Friday with the possibility of working one weekend a month. We are also looking for a dedicated weekend nurse to work Friday - Monday. We serve patients in both St. Charles county and St. Louis county, so we can give you a route that is convenient to you!

Duties include but are not limited to:

• Conduct comprehensive patient assessments to evaluate health status and develop individualized care plans.
• Provide direct patient care, including administering medications, monitoring vital signs, and performing necessary medical care such as wound care and catheter insertion and care.
• Perform and complete start-of-care, recertification, resumption-of-care, and discharges to OASIS and Agency specification.
• Educate patients and families on health management, treatment plans, and preventive measures.
• Maintain paperwork, such as med lists, visit notes, communication with MDs, to State and Agency standards is vital.

Preferred candidates are highly motivated RNs with the ability to work independently, have excellent communication, paperwork, and time management skills, and want to make a difference on a daily basis for the population we serve.
