Status: Exempt
Working Conditions: Works in an office and community setting on a regular basis. May require prolonged standing and walking, climbing stairs and lifting up to 50 pounds. May be required to serve as after-hours, crisis on-call periodically throughout the month that may include weekends and holidays.
Position Summary: Manages a predetermined caseload and responsible for the direct administration and implementation of Transitional Living services to older youth and young adults residing in community scattered site housing. Present life skills curriculum and education in a one on one or group format. May include supervisory duties of Youth Care Workers or program Specialists.
Essential Job Functions
- Ensure that regular home and community visits are completed and documented per contract.
- Ensure safety of youth and environment per contract standards.
- Implement approved programming and curriculums to meet contracted standard.
- Coordinate therapeutic and community services as needed.
- Assist youth to develop a transition plan portfolio that includes all needed adult documentation, resources, and skills acquisition.
- Complete all required intake, assessment and goal planning duties.
- Serve as after hours, crisis On-Call as assigned.
Requirements
Bachelor’s degree in human services field required. Must be over the age of 21. Experience in human services field is beneficial.
Other Requirements: Must hold a valid driver’s license, have and maintain a risk-free driving record. Must be willing to undergo initial and periodic child abuse/neglect, criminal history, sexual offender, and childcare/elder care disqualification list screenings. As requested, must submit fingerprints for review by the state of your employment and Federal Bureau of Investigation (FBI). Must undergo a pre-employment drug screen and a pre-employment physical with the results showing no evidence of communicable disease.
