Job Responsibilities
- Collaborate with therapist on observations, progress towards goals, and recommended updates to plan of care and discharge.
- Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.
You will have the opportunity to work in multiple settings – Assisted Living, Independent Living, Home Health, and Outpatient – using your strengths to serve our diverse population.
