PHS Rehabilitation is seeking a Part Time Speech Language Pathologist to serve in our assisted living and independent living communities in Menomonee Falls and Germantown, WI.

Responsibilities
As a Speech Language Pathologist you will be responsible to :
Conduct a comprehensive assessment & evaluation using subjective and objective measures.
Develop treatment plans and programs in accordance with physician orders.
Provide quality treatments, actively progressing goals to achieve highest quality of life.
Conduct screens and provide education to residents / clients, families, caregivers, and staff.
Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
Provide accurate and timely documentation & billing in electronic medical record.
Demonstrate understanding and knowledge of payer sources and regulatory requirements.
Complete and document supervisory visits per regulations.
Communicate effectively with residents / clients, families, caregivers, physicians, referral sources, and interdisciplinary team.
Qualifications
Graduate from an accredited Speech Language Pathology program.
Current licensure in the state(s) of practice as required.
Demonstrated compatibility with PHS's mission and operating philosophies.
Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
Benefits
Full-time benefits-eligible employment begins at a minimum of 30 hours per week :
