Qualifications
- Active Registered Nurse licensure (in state of job location) required
- Active CPR certification preferred
- One to two years’ experience working in Geriatric or Long-Term Care setting preferred
- Ability to assess and effectively communicate with residents
- Must have strong initiative and be able to work in a fast-paced environment
- Ability to problem solve and communicates effectively
- Ability to adjust work schedule at times to meet admissions needs
- Demonstrated ability to provide education to patients and/or care providers for disease and condition management post discharge
- Responsible for managing the coordination of the daily admissions to the Health Center.
- Assists with family and patient orientation to the neighborhood, initiating clinical pathways, and scheduling family, patient, and interdisciplinary team meetings.
- Collaborates with the interdisciplinary team regarding assessment completion, Discharge Planning, Documentation and patient education.
- Collaborates with Nursing Supervisor and other Health Center staff regarding overall communication and admissions updates.
- Manages the education process for the patient and family regarding services, admissions, and discharge process.
- Collaborates with Therapy to establish realistic discharge goals based on patient’s needs and abilities.
- Initiates discharge process on admission.
- Ensures safe discharge to home.
