For over 50 years PSS has performed vital and important work through its ten centers, two apartment buildings and affiliated programs. The agency touches the lives of over 2,000 people each day. As the Social Worker, you will be responsible for coordinating supportive services for residents in one of the many communities that PSS serves. The Social Worker’s role is vital in ensuring effective supportive services are put in place that allows the residents to remain independent, active and to thrive in their home and community.New York Connects : The NY Connects program is a statewide initiative designed to help older adults and individuals with disabilities (across the lifespan) connect to long-term services and support. As part of this initiative, we offer phone-based support and in-person assistance to guide individuals in accessing essential resources, such as healthcare services, benefits, and community programs. We provide a strengths-based, person-centered approach that respects each individual's goals and promotes independence and aging-in-place. Our goal is to help community members navigate and access services in a seamless, non-judgmental, and culturally competent manner, regardless of financial status.This position reports to the Associate Director of NY Connects.PRIMARY RESPONSIBILITIES : Develop and maintain relationships with community-based organizations, healthcare providers, senior centers, and local advocacy groups to promote the NY Connects program.Conduct public education sessions and outreach events to raise awareness of available services for older adults and individuals with disabilities in Brooklyn.Coordinate and execute outreach strategies to increase awareness of long-term care options, support services, and public benefits available through the program.Provide objective and unbiased information about long-term care options, community supports, and public services to individuals, family members, caregivers, and professionals.Assist community members in understanding and applying for services and benefits, including Medicaid, SNAP, and HEAP.Conduct screenings and assessments to determine eligibility for services both over the phone and in-person.Facilitate referrals to appropriate services and provide follow-up to ensure access to services and client engagement has been fulfilled.Use a strengths-based, person-centered approach to support individuals in achieving their care goals and promoting independence.Communicate with clients in a respectful and non-judgmental manner, ensuring cultural sensitivity and understanding.Advocate for clients who may encounter barriers in navigating community resources, government programs, or healthcare systems.Provide ongoing care coordination for clients transitioning through various life stages or situations (e.g., hospital discharge, nursing home transitions, aging-in-place).Maintain accurate case records and track outreach efforts using the NYC Aging community member database and other reporting tools.Prepare periodic reports on outreach activities, service referrals, and community engagement outcomes.Work closely with the Program Director and team members to strategize and enhance outreach initiatives.Participate in regular team meetings, training, and professional development sessions to ensure alignment with organizational goals and NYC Aging Program Standards.Perform additional tasks or responsibilities as requested by the Program Director or management team.QUALIFICATIONS AND EXPERIENCE : Bachelor’s degree (BSW, BS, BA) required.At least 1 year of experience in social services, human services, or a related field, preferably with older adults or individuals with disabilities.Language proficiency : English (Required); Candidates fluent in Spanish, Russian, Mandarin / Cantonese, or Haitian Creole are encouraged to apply!Strong written and verbal communication skills.Ability to engage and build rapport with a diverse group of clients, family members, and community organizations.Proficiency in Microsoft Office Suite (Word, Excel, Outlook).Knowledge of community resources, long-term care programs, and benefits such as Medicaid, SNAP, and Medicare is preferred.PROFESSIONALISM AND OTHER QUALITIES : Ability to work independently, manage time effectively, and handle a variety of tasks simultaneously.Familiarity with cultural competence and experience working with diverse populations.Ability to travel independently around Brooklyn using public transportation is required.This position is hybrid, with the ability to work from home 2-3 times a week, with occasional evenings and weekends required.EEOC and DEI Statement : At PSS we are committed to providing equal employment opportunities to all employees and applicants for employment regardless of race, color, religion, sex, national origin, age, disability, or any other legally protected status.
