About the Role:
We are seeking an enthusiastic and experienced Physical Therapist to provide high-quality, 1:1 therapy services to our patients in their homes. As a member of our clinical Home Health Team, you will work independently while receiving support and guidance from our experienced professionals.
Key Responsibilities:
- Provide individualized physical therapy services to patients in their homes.
- Conduct thorough assessments and develop personalized treatment plans.
- Collaborate with our multidisciplinary team to ensure comprehensive patient care.
- Maintain accurate and timely documentation of patient progress.
Requirements:
- Bachelor's or Master's degree in physical therapy.
- A minimum of one year of experience as a licensed physical therapist.
- Preferred experience in community/home health settings.
