The LPN Assessment Coordinator is responsible for ensuring quality care and services for the community residents. The role is supervisory with oversight of both professional and non-professional team members in the nursing department. In this role, the successful candidate will be responsible for the clinical reimbursement processes and procedures for Medicare and Managed Care systems and the RAI process under OBRA 1987 to support the organization's goals of providing high quality care and quality of life by using good clinical practice within a compliant and ethical culture.

What we are looking for:

We need team members who will help our residents by supporting their daily living needs and by developing trusting, caring relationships. Successful candidates for this role are required to:
  • Be a Licensed Practical Nurse with a valid license from the State of PA, in good standing. Long term care experience is preferred;
  • Have at least 2 years' experience completing MDS and facilitating interdisciplinary care planning;
  • Be able to effectively interact with residents, resident families, and team members;
  • Have working knowledge of Internet Software, Spreadsheet Software, MDS, and Word Processing Software.

Supervisory experience in a skilled nursing community is preferred.
