Envisage provides older adults with aging services and in-home care community support that is designed to assist those that want to age in their homes and avoid moving to a nursing home or assisted living community. Envisage supports our Members in achieving their goal of aging successfully in the place they call home.
The Envisage Personal Wellness and Care Coordinator provides individualized plans of care for each Member using effective and efficient case management techniques. The role must maintain professional relationships with Members, physicians, Extended Care Partners, and all other disciplines involved in Member care. Successful candidates will demonstrate detailed and up to date clinical assessment skills and knowledge of available resources for older adults as well as the ability to serve as a wellness coach to help people age successfully in the place they call home.
