Position Summary

The Service Coordinator works as a part of an interdisciplinary team in order to provide a comprehensive, community-based service to Young Adults ages 18-27 who have significant mental health needs as well as a history of involvement with welfare or juvenile justice systems. The Service Coordinator will monitor service delivery and progress towards goals, demonstrate problem resolution and assist the youth in building an informal support network.

Essential Responsibilities

  1. Provides professional coordination of mental health services according to the individual’s level of need to promote individuals’ mental health recovery and serves as a key member of the MTAY team, ensuring often complex services produce positive outcomes.
  2. Develops relationships with the young adults and important people in his/her life to assists with developing informal and formal relationships in the community for successful transition to independent living.
  3. Assesses individual strengths and needs in a collaborative method through individual and/or collateral interviews and reviews of social and clinical information provided by other entities.

  4. Promotes service planning efforts which result in developing, documenting, and implementing a comprehensive service plan driven by the individual.

  5. Evaluates all services received by young adults who are served by the MTAY program.

  6. Investigates new resources and communicates with prospective resources as well as maintains a catalog of community resources.

  7. Convenes and facilitates interdisciplinary service planning meetings or other related team meetings to ensure appropriateness and responsiveness of services in relation to individual needs.

  8. Ensures young adults being served needs are met through the utilization of natural supports (family, friends), community and generic services and specialized services (MH/MR, Supported Employment, OVR, D&A).

  9. Provides culturally competent services with consideration for the young adult’s racial, religious, sex, sexual orientation, age and ethnic background and identification.

  10. Advocates for and with the young adult being served to ensure responsiveness from resources.

Qualifications 

  • Clearances. State Police; FBI clearance; child abuse clearance; CPSL Mandated Reporter-Recognizing and Reporting Child Abuse training; any additional background checks/clearances required by state governing bodies.
  • Valid driver’s license and current vehicle insurance.

Working Conditions

  • Physical Demands.  Requires vision, speech, and hearing and a moderate level of physical activity including; sitting, standing, walking lifting and carrying as required.  
  • Environmental Factors. Office setting for staff meeting, supervision and completion of paperwork, in-home community-based service delivery.
  • Working Hours. Requires a minimum 40 hour work week. 
    • Scheduled work hours are based upon the needs of the consumers and families and often require nontraditional hours, including early mornings, evenings, and weekends. It is expected that the SC schedule home visits at times convenient for consumers and families.
    • 24/7 on-call when scheduled. Provides twenty-four (24) hour availability for Service Coordination individuals and/or families through phone and face-to-face contacts.  This requires independent thinking and good judgment.  Is on site for all in-patient MH commitments.  Ensures that all alternatives to hospitalization have been considered. Coordinates intervention when hospitalization is required.
