Job Title: Family Coordinator  
Reports To: Program Director
Position Type: Full-time / Part-time

Job Summary

The Family Coordinator plays a critical role in providing comprehensive support to families impacted by substance use. Through detailed assessments, needs identification, and collaborative service coordination, the Family Coordinator ensures that families access appropriate resources across social determinants of health. This role involves direct client advocacy, service navigation, and ongoing coordination with treatment and service providers to develop and deliver effective interventions. The Family Coordinator also collaborates closely with the Peer Family Support Specialist to foster a supportive, strengths-based approach for each family, ensuring their needs are met holistically and efficiently.

Duties and Responsibilities

  • Needs Assessment: Collaborate with families to identify their needs, using comprehensive assessments and screening tools to address challenges related to housing, healthcare, education, employment, substance use, and overall family stability.
  • Service Planning: Collaborate with families to develop individualized service plans based on assessment findings & the interests of the families, helping families set realistic goals and actionable steps to address identified needs.
  • Treatment Providers: Coordinate regularly with substance use treatment providers, mental health professionals, and other service agencies to facilitate integrated, family-centered support.
  • Service Coordination: Connect families with community resources, substance use treatment, recovery supports, and other essential services; facilitate referrals to external service providers when appropriate.
  • Advocacy for Families: Actively advocate for families within child welfare, treatment, legal, and social service systems, ensuring that services are accessible and responsive to their needs.
  • Data Management and Program Reporting: Maintain accurate, up-to-date records on all family interactions, assessments, and service activities, ensuring data is entered promptly into the program’s information systems.
  • Collaborative Development of New Services: Identify gaps in current resources and assist in developing new services and interventions tailored to the evolving needs of families in the program.
  • Support for Peer Family Support Specialist: Work closely with the Peer Specialist to ensure a seamless support network for families, providing guidance and assistance in service delivery.
  • Ongoing Family Communication: Establish regular check-ins with families to assess progress, adjust service plans, and respond to emerging needs.
  • Resource Development: Research and identify new services, community resources, and partnership opportunities to expand support options available to families.
  • Crisis Intervention: Provide direct support and intervention when families face immediate challenges, helping them access emergency resources and maintain stability.
  • Client Transportation: Provide transportation for clients to their initial appointments with service providers, treatment centers, or other critical resources to ensure timely access to necessary support services.

Education and Training Requirements

  • Minimum Education: Bachelor’s degree in Social Work, Psychology, Counseling, or a human services related field.

Experience, Knowledge, and Skill Requirements

  • Professional Experience: Background in social work, case management, or family support services, ideally with experience in substance use treatment, mental health, or child welfare settings.
    • Minimum of one year of experience working with families or individuals in a behavioral health setting, including roles that support substance use treatment or recovery.
  • Knowledge of Social Determinants of Health: Familiarity with the impact of social determinants on family stability, including housing, healthcare, education, and employment.
  • Advocacy and Service Coordination Skills: Strong ability to advocate for client needs and coordinate services across multiple agencies.
  • Interpersonal and Communication Skills: Exceptional ability to communicate empathetically, listen actively, and engage families with respect and cultural sensitivity.
  • Organizational Skills: Proficiency in managing multiple cases, tracking client data accurately, and maintaining organized documentation for program reporting.
  • Data Management: Experience with case management systems or databases to ensure accurate and timely data entry and reporting.
  • Cultural Competence: Awareness and respect for diverse backgrounds, values, and experiences, with a focus on racial, ethnic, and socioeconomic factors affecting family dynamics.
