Summary of Job Function: Works with Prestera programs and consumers to promote wellness activities in the office setting or community setting and includes but not limited to: exercise, eating healthy, dieting, smoking cessation activities, and having regular appointments with Primary Care Physicians.

Essential Functions:
1. Develops and implements wellness activities for Prestera programs.
2. Works with consumers on wellness activities and referrals to primary care.
3. Attends program meetings to discuss wellness initiatives within the agency.
4. Works with DHHR, primary care facilities, and other agencies to gather information for consumers to access services.
5. Leads groups, classes, and/or seminars on a variety of wellness topics.
6. Provides individualized health coaching to consumers in obtaining their health objectives.
7. Teaches others about managing their mental and physical health. Support consumers in learning how to make good choices for themselves by creating and adapting wellness objectives to overcome barriers to good mental and physical health based on needs of, and with input from consumers.
8. Works closely with Health Home team to engage consumers in appropriate services and resources and assist with coordination of care.
9. Promotes consumer engagement in therapy with clinicians, engagement with psychiatric appointments, involvement and engagement with wellness services, and engagement with primary care.
10. Completes all tracking and reporting requirements for outcomes and evaluation.
11. Maintains appropriate professional standards and provides appropriate follow-up for consumers.
12. Provides self-help recovery services (WRAP) and other peer wellness services.
13. All other duties assigned.
Job Specifications:
1. Education, Certification/License, and/or Experience
a. BA degree from an accredited four-year college or university in Psychology, Social Work, or Counseling.
b. Valid Drivers License with reliable transportation.
c. Must attain/maintain certification in Non-violent Crisis Intervention and CPR/FA during employment.
2. Knowledge, Skills, and Abilities required: Experience or prior training working with behavioral health and/or primary care setting is preferred. Must possess the ability to read, write reports, perform basic math (adding, subtracting, multiplying, and dividing), have good computer skills, and have good verbal and written communication skills.
