About the Job


The CCC service adopts a “whole-person” approach to address the client’s needs related to physical health, behavioral health, and social determinants of health. CCC involves coordination of care between the behavioral health clinician and the clinical personnel of an external provider (e.g., primary care, another behavioral health provider, hospital).

The ideal candidate will possess excellent communication skills; strong organizational and analytical skills; clinical or treatment experience or familiarity with substance abuse disorder and recovery; established contacts and relationships with local community resources for housing, jobs, job training, education, mental health and substance abuse treatment.

Job Description:
Prestige SUD Clinical Care Coordinator will work in under the SUD program manager to oversee all operations of the SUD Level 1 Outpatient and SUD Level 2.5 Day Treatment Day. The CCC shall ensure appropriate consumer referrals, authorizations, and transitions to new levels of care (LOCs). Additionally, the CCC will ensure follow-up and clinical care coordination of services and actual connection to other services for the consumer. Will facilitate managing day to day operations of the program, scheduling court hearings and follow up with CSOSA/Probation or Parole officer if consumer is mandated to receive outpatient SUD treatment, community resource management and data tracking. The CCC is also an integral member of the SUD Treatment Team(s).

CCC occurs when the practitioner, through direct face-to-face contact, telehealth/ videoconferencing, or telephone, communicates treatment needs, assessments, and treatment information to external health care providers and facilitates appropriate linkages with other health care professionals, including transitions into or from higher levels of care or institutional settings. CCC also includes treatment planning and plan of care implementation activities that are separate from the TAPS/diagnostic assessment service, when the clinician and client are meeting face-to-face or through video-conference/telehealth conference.

The Clinical Care Coordinator is responsible for ensuring that the client is at the appropriate LOC. If the client fails to make progress or has met all of their treatment goals, the Clinical Care Coordinator shall ensure timely assessment and transfer to a more appropriate LOC.

The CCC service must be documented in an encounter note that indicates the intended purpose of that particular service, the actions taken, and the result(s) achieved.

Identifies community resources that can assist in the consumers recovery, developing an understanding the process of SUD recovery and encourages consumers to take action on their own behalf toward recovery.

CCC shall be provided in certified SUD treatment programs or community settings.

Qualifications:
  • District of Columbia Qualified Practitioners of CCC are: (1) Physicians (2) Psychologists (3) LICSWs (4) LGSW; (5) APRNs (6) RNs (7) LISWs (8) LPCs (9) PAs (10) LMFT and (11) LGPCs.

Performance expectations:
  • Clinical Care Coordination encounters, Crisis Intervention, Referring consumer to other services/agencies for needs (i.e. linking consumer to primary health care physician), Completion of Diagnostic Assessment updates, Treatment planning/Plan of Care updates, Individual, Family and Group SUD Therapy, Request for Insurance Authorization additional units, Documentation in encounter notes, Familiar with EMS (Credible/ Data Wits), Completion of referrals to SUD and ACT, Discharge planning and/or transfers of consumers, Provides assistance with consumer support recovery and psychoeducation (HIV, STDs, and other infectious diseases). Must provide minimum of 8 hours weekly of SUD counseling to outpatient consumers. 
  • Must provide maximum of 30 hours weekly of SUD services including clinical care coordination to consumers. 
  • Must meet weekly quota of minimum of 38 billable hours. 
  • Familiar with DSM-IV and ASAM diagnostic criteria. Shall ensure to comply with all Federal and District laws and regulations pertaining to scope of practice, licensing requirements and supervision requirements.

Preference will be given to applicants with experience in one or more of the following areas:
  • Working with District of Columbia Residents with lower SES and with substantial exposure to the substance abuse/SUD/OUD and mental health comorbidity populations.
  • Working as a member of a substance abuse treatment team.
  • Direct experience with the array of treatment populations and social services supports: roofless/homeless, unstable housing, addiction treatment services, supportive employment, SNAP/SSI/Medicaid recipients, criminal justice involved.
  • Substantial prior experience in working with duel comorbidity populations in behavioral health, SMI/SED, IDD, physical health and substance use.
