POSITION SUMMARY
The Psychiatric Rehabilitation Program Direct Care Worker will provide client services, as well as support for families in hopes to improve the social and psychological functioning of clients and their families. The Psychiatric Rehabilitation Program Direct Care Worker is ultimately maximizing the client and family’s well-being and overall functioning. This position will report to the Psychiatric Rehabilitation Program Rehabilitation Specialist. Prestige Healthcare Resources Inc is looking for a Direct Care Worker to serve as an integral member of our team by providing goal-oriented individualized rehabilitative services that enable clients to develop skills that enhance the quality of their lives. Rehabilitative services are developed in partnership with the client, their support system including parents/guardians (if applicable), and mental health professionals in key life domains such as self-care skills, social skills, independent living skills, occupational and/or educational goals, spiritual and leisure activities to facilitate community integration.DUTIES AND RESPONSIBILITIES
- Assist residents with community integration, including programs, services, entertainment, socialization, and transportation.
- Provide direct services to clients in various settings (home, community, office) a minimum of 4-6 times monthly, adjusting based on client needs and goals.
- Maintain accurate documentation of all activities and interventions in the client's electronic health record, providing feedback and reports to professional staff.
- Assist in the development and implementation of clients’ Individual Rehabilitation Plans (IRPs).
- Plan, organize, and conduct rehabilitation assessments, evaluations, and interventions, ensuring the client's family and/or legal guardian (when permitted) participate in the development of the IRP.
- Model and assist in developing coping and communication skills.
- Teach clients independent living skills, addressing service gaps, and promoting community integration.
- Conduct rehabilitation groups and activities to meet clients' rehabilitation and support service needs.
- Maintain consistent documentation for each client, including contact/progress notes, monthly summaries, and quarterly reviews.
- Collaborate monthly with the therapist for each client on your caseload and document this in the monthly summary.
- Attend mandatory monthly staff meetings.
- Evaluate and review individualized rehabilitation plans, assess client progress, and recommend modifications or termination as needed.
- Facilitate transitions, discharge, admission, and clinical supervision to ensure continuity of care and follow-up.
- Other duties as assigned.
EXPERIENCE AND QUALIFICATIONS
- High School diploma or GED required
- Associate or bachelor’s Degree in a Human Services related field preferred
- At least one year of human services experience preferred
- Excellent verbal, interpersonal, and written communication skills
- Strong time management skills
- Patience and willingness to engage in ongoing professional development
- Demonstrated knowledge and skills working with colleagues in a multicultural setting
- Ability to work in unpredictable, crisis response, and team-oriented environments with emotional intelligence
- Ability to maintain strict confidentiality in accordance with federal and state laws regarding current and former clients, their families, and referents
- Self-directed with the ability to work independently
- Proficient computer skills in Microsoft Outlook and other standard software applications.
- Legally authorized to work in the U.S.A.
