POSITION SUMMARY
The Wellness Director RN partners with Health Services leadership to oversee the clinical aspects of the
health services department for assigned community and ensures that resident
care needs are met by completing assessments, coordinating resident care,
managing health services systems, and delegating nursing tasks. Promotes
a positive physical
and psychosocial environment for the residents.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Creates a resident-centered approach
to the planning and delivery
of services to residents, supporting residents’ rights to
participate in decision-making and treating residents with dignity and respect.Performs
assessments on health, functional and psychosocial status of residents.
Participates in the development of
individualized service plans. Proactively manages care and services for each
resident, maximizing the resident’s opportunity to remain
in the assisted living environment. Problem-solves as part
of the management team to case manage and meet the needs of residents.Assesses or evaluates residents
with reported changes
of condition. Provides
direction, service planning,
and coordination of care to meet the residents’ changing needs.Collaborates
with the management team on the move in of prospective residents, taking into
consideration the needs of the prospective resident,
current community acuity,
and staffing levels. Provides in-person pre-move in
assessments, if necessary.Ensures
that resident care needs are met by coordinating services with other
professionals, including physicians, discharge planners, nurses, home health
agency staff, case managers, hospice, and state agency personnel. Oversees the arrangement of ancillary services
such as pharmacy, podiatry, lab, and
x-ray as needed.Delegates
nursing tasks as allowed under applicable state nursing laws and practice acts,
communicating regularly with residents, their families, and their health
care practitioners.Oversees the documentation and communication of resident care and services.
Assists the leadership team to ensure resident records
are completed in a timely manner and maintained according to company policies
and state regulations.Maintains and oversees medication services including staff training, staff
oversight, pharmacy
coordination, medication room, and medication systems.Partners in the investigation and root cause analysis
of incidents. Collaborates with the management
team on the development and implementation of appropriate interventions.Assists in the management of Infection Control.
Assists with providing training to staff
on infection control
practices, investigates, and reports incidents of infection and tracks
infection data.Provides on-call
support and phone triage for team members’
questions regarding resident
concerns.Participates in leadership team by actively
contributing in decision
making, incident follow up, building and company-wide initiatives and attending
leadership team meetings
such as daily stand up, Systems Monitoring and Resident Tracking (SMART), Quality Assurance Performance Improvement (QAPI), and other
meetings as required.Acts as an ambassador of Prestige by representing communities in a positive
manner to outside agencies and participating in
community events and social functions.Maintains up-to-date knowledge of all rules,
regulations and policies pertaining to resident care. Responsible to stay
informed of industry and health care trends and standards, applying new
knowledge to meet the changing
needs of residents. Seeks out additional expertise and advice
when needed to assure residents’ needs are met and proper practices
followed.Performs other duties
as assigned.

QUALIFICATIONS
AND EXPERIENCE
Must have current, unencumbered RN license in the state of practice.
Delegation experience in the
state assigned preferred. Experience in geriatric nursing, medication
administration, case management, and training and staff development preferred.
EDUCATION
Registered Nurse license with a Bachelor
of Science in Nursing or Associate Degree
