Home Health LVN

Overview
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The LVN will provide high-quality nursing care to patients in various settings, including home community care and clinical environments. This role requires a strong commitment to patient health, safety, and comfort, while adhering to established protocols and standards of care.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Assist in the care of patients with infectious diseases, ensuring strict adherence to infection control protocols.
• Perform procedures such as spinal taps and sonography as needed.
• Participate in discharge planning to ensure a smooth transition for patients leaving the facility.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Monitor patient conditions and report any changes or concerns to the supervising nurse or physician.
• Provide education and support to patients and their families regarding health management and community resources.

Skills

• Proficient in medication administration and understanding of pharmacology.
• Knowledgeable in infectious disease care and infection control practices.
• Familiarity with electronic health record systems, particularly Athenahealth.
• Experience in home community care settings is a plus.
• Strong communication skills for effective interaction with patients, families, and healthcare professionals.
• Ability to perform specialized procedures such as spinal taps and sonography.
• Understanding of ophthalmology practices is beneficial.
• Excellent organizational skills for managing multiple patient needs efficiently.

Pay: From $40.00 per hour
