What are the responsibilities and job description for the Case Manager Residential Treatment Center position at Prestige Homecare?
General: We are a residential treatment facility of a youth group home for girls. The Case Manager will provide/complete admission assessments or any other evaluation of a child placement. The case manager will be responsible for the initial service plan, updates and reviews, discharge or transfer summaries. The case manager will be responsible for approving any restrictions that be imposed on a child in care, approving any restrictions to communication and visitation with the child’s family, approving any restrictions to a particular room or building that are imposed on a child and make monthly reports. Responsible for ensuring the children in the program appropriate treatment adequate in both frequency and duration based upon the needs of each individual child.
Supervisor: Treatment Director/Administrator/Executive Director
Qualifications:
1) Case Manager must have a bachelor’s degree; bachelor’s degree may be waved with 4 years of case management experience, with at least 1 years’ experience in a general residential operation, group home, or foster home. Also waived with 6 years of General Residential Operation experience, group home or foster home.
2) Must be able to successfully pass a criminal history/abuse neglect background check.
3) Some typing skills required.
Specific Duties:
- Shall be responsible for assisting with the implementation of Policies and Procedures that support the goal of reducing and ultimately eliminating the use of restraints in the facility.
- Shall be responsible for participating in ongoing evaluation of Program Policies and Procedures to assess the appropriateness of these in support of the goal of reducing and ultimately eliminating the use of restraints in the facility, including regular review of data on the use of Restraints and “Near-Misses” in the facility.
- Carries a caseload of children for which he or she will be responsible for designing treatment strategies, staffing with the psychiatrist, writing treatment/service plans, doing child assessments/recommendations, and other important documentation.
- Reviews and approves the incident reports and Restraints of residents at “Prestige Homecare”, including participation in Formal Administrative Debriefings of these events.
- Completes regular audits of documentation of services provided to ensure compliance with agency Policy and Procedures as well as Licensing and Monitoring standards.
- Coordinates communications with caseworkers, probation offices, and family members.
- Assist in admission procedures as assigned.
- Provides supervision to direct care staff who participate in treatment programs for children.
- Act as the liaison between the Independent School District and Prestige Homecare Center.
- Responsible for maintaining case files in compliance with licensing standards, Youth for Tomorrow standards and any other agency “Prestige Homecare” contracts with.

- Provides assistance with data entry in the DFPS PMET System and other databases, as needed.
- Ensures children receive nurturing care.
- Assist in transportation of children/assist in meal prep.
- Assist in dissemination of medication to child.
