Job Summary:
We are seeking a compassionate and dedicated Direct Care Worker to provide high-quality care and support to individuals with developmental disabilities, hospice care needs, or other health conditions. The ideal candidate will have a strong background in healthcare and be able to assist with vital signs monitoring, medication administration, and patient care plans. Responsibilities:
- Assist clients with daily living activities, including bathing, dressing, and grooming
- Monitor and record vital signs, such as blood pressure and pulse
- Administer medications as directed by the healthcare team
- Develop and implement individualized care plans to meet each client's unique needs
- Provide emotional support and companionship to clients
- Assist with household chores and meal preparation
- Collaborate with social workers to address clients' social and emotional needs
- Monitor and report any changes in a client's health or behavior
- Provide hospice care to clients nearing the end of life
- Maintain a safe and clean environment for clients
- Work in collaboration with other healthcare professionals, including nurses and home health aides
Requirements:
- A high school diploma or equivalent
- Experience working with individuals with developmental disabilities, hospice care needs, or other health conditions
- Knowledge of vital signs monitoring and medication administration
- Experience developing and implementing individualized care plans
- Ability to lift, bend, and move clients as needed
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and empathetic nature
- Familiarity with social work principles and practices
- Ability to maintain confidentiality and adhere to HIPAA guidelines
- Current certification in CPR and First Aid, preferred
- Ability to work flexible hours, including evenings and weekends
- Valid driver's license and reliable transportation (for certain roles)
- Ability to pass a background check and maintain any necessary certifications or licenses.
