Direct Care Worker (DCW)

Job Summary:

We are seeking a compassionate and dedicated Direct Care Worker to provide high-quality care and support to individuals with developmental disabilities, hospice care needs, or other health conditions. The ideal candidate will have a strong background in healthcare and be able to assist with vital signs monitoring, medication administration, and patient care plans. This role is perfect for individuals who are passionate about helping others and have a commitment to delivering exceptional care.

Responsibilities:

- Assist clients with daily living activities, including bathing, dressing, and grooming
- Monitor and record vital signs, such as blood pressure and pulse
- Administer medications as directed by the healthcare team
- Develop and implement individualized care plans to meet each client's unique needs
- Provide emotional support and companionship to clients
- Assist with household chores and meal preparation
- Collaborate with social workers to address clients' social and emotional needs
- Monitor and report any changes in a client's health or behavior
- Provide hospice care to clients nearing the end of life
- Maintain a safe and clean environment for clients
- Work in collaboration with other healthcare professionals, including nurses and home health aides

Requirements:

- A high school diploma or equivalent
- Experience working with individuals with developmental disabilities, hospice care needs, or other health conditions
- Knowledge of vital signs monitoring and medication administration
- Experience developing and implementing individualized care plans
- Ability to lift, bend, and move clients as needed
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and empathetic nature
- Familiarity with social work principles and practices
- Ability to maintain confidentiality and adhere to HIPAA guidelines
- Current certification in CPR and First Aid, preferred
- Ability to work flexible hours, including evenings and weekends
- Valid driver's license and reliable transportation (for certain roles)
- Ability to pass a background check and maintain any necessary certifications or licenses.

Job Types: Full-time, Part-time

Pay: $12.00 - $15.00 per hour

Education:

• High school or equivalent (Required)

Experience:

• Patient monitoring: 1 year (Preferred)
