Preston Taylor Community Health Centers Inc , is composed of a team of dedicated health care professionals that have at heart the mission of the organization of providing comprehensive primary health care services to the residents of the four-county service area of West Virginia in Preston, Taylor, Tucker, and Grant Counties regardless of their ability to pay. This is accomplished through the provision of primary care services that are culturally sensitive and relevant to the needs of the community.
In supporting the delivery of activities deriving from 6 Community Health Care Medical Centers, 2 Pharmacies, 2 Dental Centers, 1 Optometry Office, and a School Based Health Center, we are seeking a Care Coordinator to join the organization. This position will work from the Medical Center of Taylor County in Grafton, WV. The Care Coordinator will collect and analyze data integrity and consistency of EHR documentation and other QA/QI evaluations. This position will assume advocacy and accountability for an identified group of providers as part of the PCMH program.
Primary Responsibilities:
· Collaborates with the medical staff and other disciplines responsible for patient care to develop, implement, monitor, and evaluate appropriate clinical care or other services to meet the needs of patients.
· Identifies patients to be case managed, assesses patient’s care requirements, modifies or coordinates modification of patient care and intervenes as necessary.
· Assists with monitoring of quality improvement elements including establishment or program measures and implementation of action plans.
· Monitors and provides assistance with quality assurance and compliance functions.
· Works with managers/Providers to address trends that affect the agency’s outcome and process measures noted during QA review.
· May be required to work at other PTCHC sites as needed.
Qualifications
· Current WV LPN or RN license is required, as well as, CPR and First Aid Certification
· Graduate from an accredited school of nursing. At least 10 years of health care, clinical, management operations experience preferred.
- Strong computer skills and proficiency with electronic health records, must be proficient in Excel
- Strong communication skills and knowledge of the communities of which we serve
- Must be well organized with a strong attention to detail and accuracy.
